Try papers by Gouenou Coatrieux and Henri Maître. 

First, (fragile) watermarking in medical imaging should be reversible.
So the main problem may not be in technical issues, rather in designing
an accurate protocol in which watermarking is a simple tool among
others. 

If you can ensure that doctors will watch the true unmodified images,
then I think legal issues disappear. 

Not that sure it helps...
